Out of network & self-pay patients:

 Cultivate is proud to be in network with many insurance plans. If your insurance is not accepted at Cultivate Psychiatry + Wellness or you do not have insurance,  you can engage in care with Cultivate as a "self-pay" patient. Cultivate is happy to offer our commercially insured self-pay patients a superbill to submit to your insurance plan for out of network reimbursement. Self-pay fees are payable via cash, card, FSA, or HSA funds and are due at time of service.
